This form is for parents and guardians. We collect your email, the event, and — if you choose to provide them — your child's grade, teacher, and name, along with the photos you upload. These are used only to select images for the Kildeer Countryside PTO yearbook, are stored in the PTO's Google Workspace, and are visible only to PTO yearbook volunteers. We don't sell or share them, and this page uses no advertising or analytics tracking. Photos may include other students; every yearbook photo is checked against the school's opt-out list before publication. Photos that aren't selected are deleted after the yearbook is published.
